1) HEALTH PROTOCOL

  • Please, fill the SCHEDA PERSONALE DI RISCHIO (SPR) form with the help of your scientific activity supervisor, attaching all the material safety data sheets (MSDS) of the chemicals and biological materials employed in your activity. Complete this form with all the required signatures (Director of Bio@SNS, Scientific Supervisor and Chief Operating Officer).
  • The scientific supervisor verify that the final SPR is well and fully filled and that all the copies of the original form (one for the Centre, one for the SNS, one for the worker) are ready before the health examination: remember that the doctor can refuse the exam if the SPR is not well or fully filled.
  • Fill the form for medical examination
  • Bring to the doctor the original hard copy SPR form, a copy to the Bio@SNS admin and keep one for yourself. The SPR is supposed to be signed by all the subject indicated in the last page (subject, scientific supervisor, Lab director, executive officer).
  • Bring back to SNS admin (and eventually also to the belonging admin if different) a copy of the document received from the doctor, the so called “IDONEITA’ ALLA MANSIONE LAVORATIVA”, the certificate of medical fitness.
  • In case of  “IDONEITA’ CON PRESCRIZIONE”, a partial medical fitness certification, a new health exam will held after the clinic exams and hopefully finally released a full medical fitness certificate. Both the partial and full certificates have to be delivered to Segreteria Amministrativa Bio@SNS
  • If you change your scientific activity during your stay at Bio@SNS, ask to your supervisor to present a new or modified SPR and ask Segreteria Amministrativa Bio@SNS for a new health exam.
